Job summary

Giving Home Health Care is seeking an experienced RN Case Manager to lead clinical oversight and ensure high-quality, patient-centered care across care teams. The role supports Care Team Directors, maintains regulatory compliance, and mentors nursing staff in Texas.

Responsibilities include reviewing plans, managing documentation, and auditing for quality while coordinating with interdisciplinary teams. Travel to patient homes is expected as part of supervision and service delivery.

Qualifications

• Current, unrestricted RN license in TX.
• Minimum of three years of nursing experience.
• Strong knowledge of clinical documentation standards, patient assessment, and care planning.
• Excellent organizational, communication, and critical thinking skills.
• Ability to work independently while collaborating with interdisciplinary teams.
• Proficiency in EMR systems and Microsoft Office.

Responsibilities

• Provide clinical oversight and guidance to care teams.
• Review and ensure accuracy of Plans of Care and LMNs.
• Audit nursing documentation for quality and compliance.
• Supervise visits for patients without an RN or with an LPN.
• Coordinate care and address high-risk or red-flag cases.
