AccentCare, Inc. is seeking an RN Case Manager to oversee patient care plans from admission to discharge, coordinating with nurses, LPNs, and CHHAs to ensure high-quality care. The role focuses on evaluating needs, coordinating services, and guiding clinical decisions within the patient’s plan.

The position requires an RN license, CPR certification, and at least one year of RN experience, with a Monday–Friday, 8:00 a.m. to 5:00 p.m.

Qualifications

• Graduate from an approved school of professional nursing and licensed to practice as a registered nurse in the state of operation.
• Minimum of one year of experience as an RN.
• Current CPR Certification required.

Responsabilités

• Assess the status of nursing needs of assigned patients and overall management of the patient's care plan.
• Complete initial patient assessment upon admission, including appropriate patient history.
• Select appropriate nursing diagnoses for patient care.
• Assess medications to identify potential drug therapy issues and adverse reactions.
• Establish realistic, measurable goals aligned with diagnosis.
• Inform the physician of assessment results and recommended interventions.
• Implement nursing interventions within the Nurse Practice Act and established goals.
• Re-evaluate patients' nursing and clinical needs as required.
• Document and submit assessments, progress, and discharge notes timely.
