Job summary

AccentCare, Inc. is hiring an RN Case Manager to oversee patient care plans from admission through discharge, ensuring quality patient care.

The role blends direct and indirect patient care and guides RNs, LPNs, and CHHAs in delivering the patient's plan while coordinating interdisciplinary needs. Qualifications include graduation from an approved nursing program, an active RN license in the operating state, at least one year of RN experience, and CPR certification.

Qualifications

• Graduate from an approved school of professional nursing and currently licensed to practice as a registered nurse in the state of agency operation.
• A minimum of one year of experience as an RN
• Current CPR Certification required

Responsibilities

• Assess the status of nursing needs and overall management of the patient’s care plan.
• Complete initial patient assessment upon admission, including patient history.
• Select appropriate nursing diagnoses.
• Assess medications for ineffective therapy, adverse reactions, or allergies.
• Establish realistic, measurable goals aligned with diagnosis.
• Inform physician of assessment results and interventions.
• Implement nursing interventions within the Nurse Practice Act.
• Re-evaluate patients’ clinical needs as required.
• Document and submit assessment, progress, and discharge notes timely.
• Inform physician and staff of changes in patient condition.
