AccentCare is hiring an RN Case Manager to manage patient care plans from admission through discharge, guiding nurses, LPNs, and CHHAs to implement the plan of care. The role emphasizes direct and indirect patient care and multidisciplinary coordination to ensure quality outcomes.

The ideal candidate will have RN licensure in the state of operation and at least one year of RN experience, with CPR certification. The position is full-time with standard business hours and on-call responsibilities.

Formación

• Graduate from an approved school of professional nursing and currently licensed as an RN in the state of operation
• Minimum of one year of experience as an RN
• Current CPR Certification required

Responsabilidades

• Assess the status of nursing needs and overall management of the patient’s care plan
• Complete initial patient assessment on admission, including patient history
• Select appropriate nursing diagnoses and establish measurable goals
• Assess all medications for interactions, side effects, allergies, and contraindications
• Inform the physician of assessment results and recommended interventions
• Implement nursing interventions within the Nurse Practice Act and care plan
• Re-evaluate patients’ nursing and clinical needs as required
• Document assessments, progress, and discharge notes timely
