AccentCare, Inc. is seeking a full-time RN Case Manager to guide patient care from admission through discharge and ensure quality outcomes. You will coordinate direct and indirect care, lead nurses and aides, and align interdisciplinary teams to the patient’s plan of care.

The role requires current RN licensure in the state of operation, CPR certification, and at least one year of nursing experience. Compensation is $71,000-$79,000 annually, with a per-point payment structure.

Qualifications

• RN licensure in the state of operation.
• CPR certification required.
• Minimum of one year of RN experience.

Responsibilities

• Assess and manage patients' care plans from admission through discharge.
• Lead staff nurses, LPNs, and CHHAs in delivering the plan of care.
• Coordinate interdisciplinary needs and allied health clinician involvement.
• Document assessments and discharge notes in a timely manner.
