Job summary

The role includes leading RNs, LPNs, and CHHAs to deliver individualized plans and coordinate care, with a per-point compensation model and an annual estimate of 87,000 to 102,000 dollars.

Qualifications

• RN licensed to practice in the state of operation.
• Minimum of 1 year of RN experience.
• Current CPR Certification required.

Responsibilities

• Assess nursing needs and manage the patient care plan.
• Complete initial patient assessment at admission with patient history.
• Identify appropriate nursing diagnoses and ensure interventions.
• Monitor medications for effectiveness and adverse effects.
• Document progress and discharge notes promptly.
• Communicate changes in patient condition to the physician.
