Job summary

AccentCare is seeking a Registered Nurse Case Manager to oversee patient care plans from admission through discharge. You will lead a care team and coordinate services to ensure high-quality care in Warren & Claiborne Counties, MS.

The role requires RN licensure in the state of operation, at least one year of RN experience, and CPR certification. Schedule is typically Monday–Friday with on-call responsibilities, and compensation includes a $5,000 bonus package.

Qualifications

• Must be a graduate of an approved school of professional nursing.
• Currently licensed to practice as an RN in the state of operation.
• Minimum of one year of RN experience; CPR certification required.

Responsibilities

• Assess nursing needs and manage the patient’s care plan.
• Complete admission assessment including history.
• Identify appropriate nursing diagnoses and goals.
• Review medications for safety and effectiveness.
• Inform physician of assessment results and recommended interventions.
• Implement nursing interventions and document progress.
• Re-evaluate patient needs as required.
• Document assessments, progress, and discharge notes timely.
• Communicate changes in patient condition to the care team.
