Job summary

AccentCare, Inc. is seeking a Registered Nurse Case Manager to oversee care plans from admission to discharge in the Oakdale area. The role directs nurses and CHHAs, coordinating interdisciplinary needs to ensure high-quality patient care.

The position requires RN licensure, at least one year of RN experience, and CPR certification, with pay stated between $87,000 and $102,000 annually, plus per-point compensation. Schedule is Monday to Friday, 8:00 a.m. – 5:00 p.m.

Qualifications

• Graduate from an approved school of professional nursing and currently licensed to practice as a registered nurse in the state of operation.
• A minimum of one year of experience as an RN.
• Current CPR Certification required.

Responsibilities

• Assessment of nursing needs and management of the patient's care plan from admission through discharge.
• Complete initial patient assessment, including relevant patient history.
• Selects appropriate nursing diagnosis and coordinates interventions.
• Reviews medications for effectiveness, adverse effects, allergies, and contraindications.
• Establishes measurable goals aligned with the diagnosis and communicates with physicians.
• Implements nursing interventions and reevaluates patient needs as required.
• Documents assessments, progress, discharge notes, and required paperwork timely.
• Informs physicians and staff of changes in patient condition and needs.
