Job summary

TruHealth in Oklahoma is seeking an experienced RN Case Manager to oversee care coordination for Medicare Advantage members. You will visit patients in person and by telehealth to ensure proper nursing care, coordinate with physicians, and develop care plans with an interdisciplinary team.

The role requires strong communication, the ability to manage multiple priorities, and a proactive mindset. A bachelor’s degree and active RN license are preferred, with CCM desirable.

Qualifications

• Proven nursing experience with geriatrics preferred.
• Experience coordinating patient care across interdisciplinary teams.
• Ability to educate patients and families on care plans.

Responsibilities

• Complete Health Risk Assessments for members as assigned.
• Update and revise care plans as needed.
• Maintain caseload and coordinate outpatient discharge planning.
• Collaborate with health care team to evaluate and adjust care plans.
• Provide patient/family education using teaching resources.
