Job summary

TruHealth is seeking a Registered Nurse Case Manager in St. Louis, MO to manage care coordination for Medicare Advantage members. You will visit patients in person and/or by telehealth, coordinate with physicians and the health care team, and support care plans across the care continuum.

The role emphasizes strong communication, adaptability, and collaboration with multiple teams, ensuring high-quality patient outcomes in a dynamic environment.

Qualifications

• Graduate from an accredited Registered Nursing Program.
• Two years’ experience in clinical nursing or rehabilitation for the geriatric population.
• Two years’ experience in managed care / case management within a managed care environment.
• Current state RN license; valid driver’s license and vehicle insurance.

Responsibilities

• Manage assigned patient caseload and complete health risk assessments.
• Collaborate with care teams to plan, implement, and revise care plans.
• Coordinate discharge planning and follow-up based on patient needs and coverage.
• Support care coordination through audits, reporting, and monitoring tools.
• Educate patients and facility staff on identified needs using available resources.
• Respond to day-to-day care coordination issues and communicate with facilities.
