Job summary

TruHealth LLC in St. Louis, MO is seeking a Registered Nurse Case Manager to lead case management strategies for Medicare Advantage members.

You will visit patients in person or by telehealth, collaborate with physicians, and coordinate with the health care team to assess and optimize care plans. The role requires a self-starter with strong communication skills and the ability to manage multiple priorities with minimal supervision while collaborating with multiple teams to improve care

Qualifications

• Current state RN license
• Two (2) years’ experience in managed care or 3-5 years’ experience performing case management within a managed care environment
• Two (2) years’ experience in clinical nursing or rehabilitation for geriatric population

Responsibilities

• Manage assigned patient caseload
• Complete health risk assessments for designated members
• Initiate, reassess and revise plans of care in collaboration with the health care team
• Evaluate, coordinate, and plan patient care in collaboration with the interdisciplinary health team
• Provide education to the patient/family and facility staff
• Coordinate outpatient discharge planning
• Participate in Managed Care audits and maintain reports to support Care Coordination
• Perform improvement projects involving data collection and audit findings
• Participate in patient care conferences and educational programs
• Research clinical questions and educate members on health options
• Respond to day-to-day issues in care coordination and communicate with facilities
• Rounding in patient care areas as needed
• Other duties as assigned
