Job summary

A healthcare provider in Oklahoma is seeking an experienced RN Case Manager to conduct patient care management and direct nursing support. The ideal candidate must possess an Oklahoma RN license, have at least 3 years of relevant experience, and be certified in CPR, BLS, and ACLS. Qualifications

• Minimum of 3 years experience required.
• Meet pre-employment credentialing clearances (Background Check, Drug Screen, etc.).
• Ability to lift up to 50lbs.

Responsibilities

• Conduct pre-visit planning for scheduled patients.
• Communicate patient needs and recommended treatments.
• Maintain and update the team's patient panel.
• Tracks and follows up on referrals.

Skills

Oklahoma or Compact / Multi-State RN License
