Job summary

GlobalHealth is seeking a Case Manager in Oklahoma to manage care for high-risk members. You will perform telephonic case management, coordinate with providers, and advocate for members’ safety and wellbeing.

You will educate members, collaborate with care teams, and participate in SNP MOC activities including HRA, ICP, ICT, and TOC processes. Remote work with state considerations may apply.

Qualifications

• Knowledge of current nursing processes, techniques, and established standards.
• Strong attention to detail and good organization and time management.
• Clear and effective written and verbal communication.
• Proficient in Microsoft Word, Excel, PowerPoint.
• Ability to work independently or as part of a team.

Responsibilities

• Conducts telephonic case management for complex, high-risk members, identifying needs and coordinating care.
• Coordinates with providers to ensure holistic, beneficial outcomes.
• Communicates with respect to culture, language, gender, age, religion, and socioeconomic factors.
• Maintains patient advocacy and crisis intervention through interdepartmental communications.
• Coordinates services for members, including community resources and care team collaboration.
• Educates members and caregivers on conditions and self-management techniques.
• Initiates and participates in SNP MOC elements: HRA, ICP, ICT.
• Completes TOC activities: post-discharge assessment, med reconciliation, follow-up access.
