Job summary

TruHealth is seeking an RN Case Manager in Mount Sterling, KY to manage care coordination for individuals enrolled in a Medicare Advantage plan. The role involves visiting patients in person or by telehealth, coordinating with physicians, and developing individualized care plans in collaboration with a multidisciplinary team.

The ideal candidate has 2+ years in clinical nursing or geriatric care, plus managed care case management experience.

Qualifications

• Minimum 2 years' experience in clinical nursing or geriatric rehab.
• 2+ years managed care experience required.
• 3–5 years’ case management in a managed care setting preferred.

Responsibilities

• Complete Health Risk Assessments for assigned members.
• Initiate, update and/or revise care plans as needed.
• Maintain a case load of patients as assigned.
• Educate patients and families using available teaching resources.
• Coordinate outpatient discharge planning based on patient needs and benefit coverage.
• Participate in audits and generate reports to support care coordination.
