TruHealth in Connersville, IN is seeking an RN Case Manager to lead care coordination for a Medicare Advantage member group. You will visit patients in person or by telehealth, collaborate with physicians and healthcare teams, and manage and revise care plans to ensure optimal patient outcomes.

The role requires strong communication, multitasking, and adaptability across teams, with a focus on education for members and staff alike.

Qualifications

• Minimum of 2 years of clinical nursing or geriatric rehabilitation experience.
• 2 years managed care experience required.
• 3–5 years’ case management in a managed care environment preferred.
