Job summary

St. David's Round Rock Medical Center is seeking an experienced Registered Nurse Case Manager to coordinate care plans, manage length of stay, and optimize resource use while delivering safe, high-quality care.

The RN CM will discharge coordinate activities and collaborate with a multidisciplinary team to ensure smooth transitions. The role requires an Associate Degree in Nursing or Nursing Diploma, with 2+ years in case management or 3+ years in clinical nursing; BSN preferred and InterQual

Qualifications

Associate Degree in Nursing or Nursing Diploma Required
Bachelor's Degree in Nursing Preferred
2+ years experience in case management OR 3+ years experience in clinical nursing Required
InterQual experience Preferred

Responsibilities

Coordinate education for patients, families, and caregivers for participation in treatment planning and family conferences
Develop individualized treatment plans reflecting the patient’s strengths and goals
Complete psychosocial assessments and discharge planning evaluations with clear documentation
Collaborate with Rehab Program Director and interdisciplinary team for seamless patient care
Guide patients and families through program orientation, discharge criteria, and insurance benefits
