Job summary

Talented Medical Solutions is seeking a Registered Nurse (RN) – Case Manager to coordinate patient care, promote quality outcomes, and facilitate timely discharge planning across the continuum. You will collaborate with physicians, patients, families, and the interdisciplinary team to craft individualized care plans and ensure regulatory and payer compliance.

The ideal candidate holds an ASN with RN licensure in the state of practice, and a BSN is preferred.

Qualifications

• ASN is required.
• RN license must be active in state of practice.
• BLS certification preferred or required per facility policy.
• CCM/ACM certification preferred.
• Recent case management or discharge planning experience preferred.

Responsibilities

• Assess patient needs and develop individualized case management and discharge plans.
• Coordinate care with physicians, nurses, social workers, therapists, and community resources.
• Facilitate appropriate utilization of healthcare services and ensure timely transitions of care.
• Monitor patient progress and revise care plans as needed.
• Educate patients and families regarding treatment plans, discharge instructions, and available resources.
• Maintain accurate clinical documentation and ensure regulatory and payer compliance.
• Advocate for patient-centered care and collaborate with the healthcare team to improve outcomes.
• Follow HIPAA and organizational policies.
