Nacogdoches Medical Center is seeking a Registered Nurse Case Manager to coordinate patient care across the continuum and to promote safe, efficient discharge planning. The role emphasizes utilization management, transition planning, and education for patients, families, and care teams.

The ideal candidate will have an RN license, 2 years of acute hospital experience, and a background in case management. This position offers comprehensive benefits and opportunities for professional development.

Qualifications

• Requires RN license in state of practice.
• 2 years acute hospital patient care experience.
• ACM certification preferred.
• Experience in case management preferred.
• Ability to coordinate multidisciplinary teams and manage complex patients.

Responsibilities

• Accurate medical necessity screening and submission for Physician Advisor review.
• Care coordination across the continuum of care.
• Transition planning assessment and reassessment.
• Implementation or oversight of the transition plan.
• Leading and facilitating multi-disciplinary patient care conferences.
• Managing concurrent disputes and referrals to other departments.
• Identifying and referring complex patients to Social Work Services.
• Communicating plan of care with patients and families.
• Collaborating with physicians, office staff and ancillary departments.
• Timely documentation in the Tenet Case Management system.
