Parkridge Medical Center in Chattanooga, TN is hiring an RN Case Manager to coordinate care across inpatient and observation patients, promote safe transitions and discharge planning with the multidisciplinary team. You will evaluate needs, manage length of stay, and ensure efficient resource utilization while maintaining high quality and patient-centered outcomes.

The role requires an active RN license and 2+ years in case management or 3+ years nursing, with InterQual experience preferred;

Qualifications

• RN license required.
• Associate degree in nursing or nursing diploma required.
• Bachelor’s degree in nursing preferred.
• 2+ years in case management or 3+ years clinical nursing required.
• InterQual experience preferred.

Responsibilities

• Provides case management services for inpatient and observation patients.
• Identifies patients at risk for adverse outcomes during care transitions.
• Performs comprehensive psychosocial, medical, discharge needs assessments.
• Reassesses clinical conditions and educates on accessing healthcare resources.
• Coordinates care plan and discharge planning with the multidisciplinary team.
• Partners with Social Services for post-acute needs and risk referrals.
• Involves patient and family in identifying needs and goals.
• Evaluates progression of care using InterQual criteria.
