Job summary

Qualifications

• RN licensed and in good standing
• Registered Nurse Diploma or equivalent required

Responsibilities

• Guide patients and families through the care journey and discharge planning
• Coordinate education for patients, families, and caregivers
• Develop individualized treatment and discharge plans reflecting patient needs
• Complete psychosocial assessments and document findings for the care team
• Collaborate with Rehab Program Director, Case Management Director, and interdisciplinary team
