Job summary

The RN Case Manager at St. David’s Round Rock Medical Center, part of HCA Healthcare, coordinates the plan of care, facilitates transitions, and promotes throughput while balancing quality and resource use.

You will work with a multidisciplinary team, educate patients and families, document assessments, and support discharge planning, with emphasis on compassionate care and adherence to policies.

Qualifications

• Associate Degree in Nursing required or Nursing Diploma
• Bachelor's Degree in Nursing preferred
• 2+ years experience in case management or 3+ years in clinical nursing required
• InterQual experience preferred

Responsibilities

• Guiding patients and families through program orientation and discharge criteria
• Coordinating education for patients, families and caregivers
• Developing individualized treatment plans reflecting patient strengths and goals
• Completing psychosocial assessments and discharge planning evaluations
• Collaborating with Rehab Program Director, Facility Case Management Director and care team
• Promoting HCA Healthcare values through patient and family interactions
