Job summary

AccentCare is seeking an RN Case Manager to oversee patient care from admission to discharge, blending direct and indirect care and guiding nurses, LPNs, and CHHAs in executing individualized plans.

The role requires RN licensure in the operating state, CPR certification, and at least one year of RN experience. Schedule is Monday–Friday with on-call weeks, and compensation is approximately $71,000–$79,000 annually.

Qualifications

• Graduate from an approved school of professional nursing and licensed in the state of operation.
• Minimum of one year of RN experience.
• Current CPR certification required.

Responsibilities

• Assess the status of nursing needs and manage the patient care plan.
• Complete initial admission assessment, including patient history.
• Select appropriate nursing diagnoses.
• Review all medicines for effectiveness, safety and allergies.
• Set realistic, measurable goals aligned with diagnosis.
• Update physician on results and interventions.
• Implement nursing interventions and monitor progress.
• Re-evaluate patient needs as required.
• Document assessments, progress and discharge notes timely.
• Notify physician and staff of changes in patient condition.
