Job summary

AccentCare is seeking an RN Case Manager in Illinois to oversee patient care plans from admission through discharge. The role combines direct and indirect care and directs nurses and aides to deliver the plan of care while coordinating interdisciplinary needs.

The RN must be licensed in the appropriate state, with at least 1 year of RN experience and CPR certification. The position is full-time with a fixed schedule and on-call requirements.

Qualifications

• RN license to practice in the relevant state.
• Minimum of 1 year of RN experience.
• Current CPR Certification required.

Responsibilities

• Assess nursing needs and manage the patient care plan.
• Complete initial admission assessment with patient history.
• Select appropriate nursing diagnoses.
• Review medicines for effectiveness or adverse reactions.
• Set realistic goals and inform physician of interventions.
• Implement nursing interventions per plan and scope.
• Re-evaluate patient needs as required.
• Document assessments, progress, and discharge notes timely.
• Inform physician and staff of changes in patient condition.
