Job summary

AccentCare in Mississippi is seeking an RN Case Manager to oversee patient care plans from admission through discharge and to coordinate direct and indirect care with RNs, LPNs, and CHHAs to deliver the plan of care.

Qualifications

• RN license in MS required.
• Minimum of 1 year RN experience.

Responsibilities

• Assess nursing needs and manage the patient care plan.
• Complete initial patient assessment on admission and collect patient history.
• Identify nursing diagnoses and monitor progress.
• Review medications for potential interactions and adverse effects.
• Set realistic, measurable goals aligned with diagnosis.
• Inform physician of assessment results and recommended interventions.
• Implement nursing interventions within Nurse Practice Act scope.
• Re-evaluate patient needs as required.
• Document assessments, progress, and discharge notes timely.
• Notify physician of changes in patient condition.
