Job summary

AccentCare in Mississippi is seeking an RN Case Manager to oversee patient care from admission through discharge, coordinating with RNs, LPNs, and CHHAs to ensure quality outcomes. The role emphasizes assessment, documentation, and interdisciplinary collaboration in line with each patient’s plan of care.

The position requires an RN license, CPR certification, and at least one year of nursing experience, with a Monday–Friday schedule and on-call responsibilities.

Qualifications

• Graduate from an approved school of professional nursing and currently licensed to practice as an RN in the state of operation
• Minimum of one year of experience as an RN
• Current CPR Certification required

Responsibilities

• Assess the status of nursing needs and manage the patient’s care plan throughout admission and discharge
• Complete initial patient assessment and gather patient history on admission
• Select appropriate nursing diagnoses and monitor medication effects for safety
• Establish measurable goals aligned with the patient’s diagnosis and outcomes
• Inform the physician of assessment results and recommended interventions
• Implement nursing interventions within the Nurse Practice Act
• Re-evaluate patient needs and adjust care plans as required
• Document assessment, progress, and discharge notes promptly
• Inform physicians and staff of changes in patient condition and needs
