Job summary

AccentCare is seeking an RN Case Manager to oversee patient care plans from admission to discharge, coordinating with staff to deliver high-quality care. The role blends direct patient contact with bedside leadership to ensure adherence to the plan of care and inter-disciplinary needs are met.

The nurse will perform assessments, establish goals, notify physicians of changes, and re-evaluate as required. AccentCare emphasizes caregiver well-being and giving patients excellent, coordinated care.

Qualifications

• Graduate from an approved school of professional nursing and currently licensed to practice as an RN in the state of operation.
• A minimum of one year of experience as an RN.
• Current CPR Certification required.

Responsibilities

• Assess nursing needs and manage the patient care plan.
• Complete initial patient assessment upon admission and collect patient history.
• Select appropriate nursing diagnoses and monitor medications for adverse reactions.
• Establish measurable goals aligned with diagnosis and communicate with physicians.
• Document assessments, progress, discharge notes, and required paperwork.
