Job summary

Qualifications

• Minimum 5 years of clinical experience and 1 year of previous Case Management experience or a UCI employee who meets the clinical requirements and has a BSN.
• Current Licensed Registered Nurse, Social Worker (MSW or LCSW), or Licensed Healthcare Therapist professional that allows for independent assessments as permitted within the scope of practice of the discipline.

Responsibilities

• The Case Manager is responsible for high level assessment and care planning for an identified group of patients which results in self-care outcomes upon discharge from the Medical Center.
• Works as part of an interdepartmental team to ensure the delivery of high quality, individualized, cost-effective patient care.
