Job summary

UCI Health, a leading academic health system in California, seeks an experienced Case Manager RN to lead high-level assessments and plan patient-centered care. You will collaborate with an interdisciplinary team to coordinate care, ensure accurate documentation, and support safe transitions across the health system.

The role requires a BSN, current RN license, and at least 5 years of clinical experience with case management.

Qualifications

• Minimum 5 years of clinical experience and 1 year of previous Case Management experience or a UCI employee who meets the clinical requirements and has a BSN.
• Familiarity with Joint Commission, CMS, CDPH and CCS requirements
• Excellent written and verbal communication skills in English
• Current RN License
• All external candidates must have a Bachelor of Science in Nursing
• Ability to manage multiple concurrent projects and maintain a work pace
• Ability to facilitate and lead interdisciplinary rounds
• Ability to establish and maintain effective working relationships across the Health System

Responsibilities

• Perform high level assessment and reassessment to set priorities, plan, organize, and implement goal-directed care.
• Collaborate with an interdisciplinary team to coordinate high quality, safe, cost-effective patient care.
• Ensure clinical information in the medical record reflects true severity of illness and improve documentation.
• Coordinate utilization review using InterQual Intensity of Service and Severity criteria.
• Advocate for patient self-determination and transitions of care.
