Job summary

UCI Health in Irvine is seeking a Case Manager to perform high-level assessment and care planning for an identified patient group, coordinating with an interdepartmental team to ensure high-quality, cost-effective discharge planning.

The role requires a Registered Nurse or licensed clinician, at least 5 years of clinical experience and 1 year in case management, familiarity with Joint Commission and CMS guidelines, and strong communication.

Qualifications

• Must possess the skill, knowledge and ability to perform duties.
• Understanding acuity levels for specific patient population.
• Demonstrate customer service skills.
• Minimum 5 years of clinical experience and 1 year of Case Management experience or a BSN.
• Familiarity with Joint Commission, CMS, CDPH and CCS requirements.
• Excellent written and verbal communication skills in English.
• Current Licensed Registered Nurse, Social Worker (MSW or LCSW), or Licensed Healthcare Therapist with independent assessment authority.
• Computer literacy and proficiency.
• All external candidates must have a Bachelor of Science in Nursing.
• Ability to manage multiple concurrent projects and workload.
• Ability to facilitate and lead interdisciplinary rounds.
• Ability to establish and maintain effective working relationships across the Health System.
