Job summary

The role requires a BSN, current RN license, 5+ years of clinical experience and case management experience, familiarity with Joint Commission and CMS requirements, and excellent English communication.

Qualifications

• BSN and active RN license required.
• 5+ years clinical experience with case management preferred.
• Familiarity with Joint Commission, CMS, CCS requirements.
• Strong English communication skills.

Responsibilities

• Perform high-level assessment and reassessment to set priorities and plan care.
• Collaborate with an interdisciplinary team to coordinate safe, cost-effective care.
• Facilitate transitions of care and ensure accurate medical record documentation.
• Utilize decision support criteria to support utilization review.
