Job summary

UCI Health in Orange County is seeking a Case Manager to lead high-level assessment and care planning for an identified patient group within an interdepartmental team. You will ensure cost-effective, high-quality care and seamless discharge planning in a complex health system.

The role requires 5+ years of clinical experience with at least 1 year in case management, a BSN, and current licensure as RN, Social Worker (MSW/LCSW), or equivalent professional with independent assessment authority.

Qualifications

• Minimum 5 years of clinical experience and 1 year of previous Case Management experience or a UCI employee who meets the clinical requirements and has a BSN.
• Familiarity with Joint Commission, CMS, CDPH and CCS requirements
• Excellent written and verbal communication skills in English
• Current Licensed Registered Nurse, Social Worker (MSW or LCSW), or Licensed Healthcare Therapist professional that allows for independent assessments as permitted within the scope of practice of the discipline
• All external candidates must have a Bachelor of Science in Nursing
