Qualifications

• Minimum 5 years of clinical experience and 1 year of prior Case Management experience.
• Familiarity with Joint Commission, CMS, CDPH and CCS requirements.
• Excellent written and verbal communication skills in English.
• Current RN License.
• All external candidates must have a BSN.
• Ability to manage multiple concurrent projects and maintain a work pace appropriate to the workload.
• Ability to facilitate and lead interdisciplinary rounds.
• Ability to establish and maintain effective working relationships across the Health System.

Responsibilities

• Perform high-level assessment and reassessment of patient/family needs to set priorities and plan care.
• Coordinate interdisciplinary care to deliver safe, high-quality, cost-effective services and transitions of care.
• Advocate for patient self-determination and culturally competent planning.
• Utilize InterQual and other criteria to review care utilization and improve documentation.
• Collaborate with physicians and care teams to ensure clear and complete medical records.
• Lead interdisciplinary rounds and coordinate resource utilization across the health system.
