Qualifikationen

• 5+ years of clinical experience and 1 year of Case Management experience or a UCI employee meeting clinical requirements with a BSN
• Excellent written and verbal communication skills in English
• Current RN license
• Knowledge of Joint Commission, CMS, CDPH and CCS requirements
• Ability to manage multiple concurrent projects and workload
• Ability to establish and maintain effective working relationships across the health system
• Proficiency with standard office equipment and computer software (MS Windows, Excel, Access, Word, PowerPoint)
• All external candidates must have a BSN

Aufgaben

• Perform high level assessment and reassessment to plan and implement goal-directed patient care
• Collaborate with an interdisciplinary team to coordinate care and transitions of care
• Advocate for patient self-determination and cultural competence in evaluations and planning
• Monitor and coordinate resource utilization within the health system
• Conduct utilization review using criteria such as InterQual Intensity of Service and Severity of Illness
• Ensure clinical information in the medical record is accurate and complete for quality documentation
