Job summary

Qualifications

• Minimum 5 years of clinical experience and 1 year of Case Management experience.
• BSN required; RN license current.
• Excellent written and verbal communication in English.
• Familiarity with Joint Commission, CMS, CDPH and CCS requirements.
• Ability to manage multiple concurrent projects and lead interdisciplinary rounds.

Responsibilities

• Perform high-level assessment and reassessment to set care priorities.
• Collaborate with interdisciplinary team to plan, coordinate, and implement care.
• Advocate for patient self-determination and cultural competence.
• Ensure medical records reflect illness severity and complete documentation.
• Coordinate resource utilization and transitions of care across the health system.
