University of California, Irvine Health is seeking a Case Manager to perform high-level assessments and care planning for a defined patient group, guiding outcomes at discharge. You will collaborate across departments to ensure cost-effective, high-quality care and support self-care goals.

The role emphasizes strong communication, teamwork, and adherence to standards. Success requires a BSN (or equivalent licensure) with clinical experience, plus familiarity with regulatory requirements and

Competenze

Minimum 5 years of clinical experience and 1 year of previous Case Management experience or a UCI employee who meets the clinical requirements and has a BSN.
Current Licensed Registered Nurse, Social Worker (MSW or LCSW), or Licensed Healthcare Therapist professional that allows for independent assessments as permitted within the scope of practice of the discipline.
Familiarity with Joint Commission, CMS, CDPH and CCS requirements.
Excellent written and verbal communication skills in English.
