Job summary

University of California, Irvine Health is seeking an experienced Case Manager nurse to join its patient-centered care team in Irvine, CA. The incumbent will perform high level assessments, plan care, and coordinate transitions with an interdisciplinary team to deliver safe, cost-effective outcomes.

Requirements include a BSN, current RN license, at least five years of clinical experience and one year in Case Management, plus familiarity with Joint Commission, CMS, CDPH and CCS requirements.

Qualifications

• Minimum 5 years of clinical experience and 1 year of previous Case Management experience or a UCI employee who meets the clinical requirements and has a BSN.
• Current RN License
• Familiarity of Joint Commission, CMS, CDPH and CCS requirements

Responsibilities

• Perform high level assessment and reassessment to set priorities and plan care.
• Collaborate with interdisciplinary team to facilitate safe, cost-effective patient care and transitions.
• Coordinate resource utilization and ensure clear and complete medical records with physicians.
• Advocate for patient self-determination and cultural competence in evaluations and planning.
