Job summary

Qualifications

• Minimum 5 years of clinical experience and 1 year of Case Management experience or a UCI employee who meets the clinical requirements and has a BSN.
• Excellent written and verbal communication skills in English.
• Current RN license and ability to manage multiple concurrent projects.
• Familiarity with Joint Commission, CMS, CDPH and CCS requirements.
• Ability to facilitate and lead interdisciplinary rounds and coordinate care.

Responsibilities

• Conduct high-level assessment and reassessment of patients and families to set care priorities.
• Coordinate with interdisciplinary teams to plan, implement, and evaluate goal-directed care.
• Advocate for patient self-determination and culturally competent planning.
• Ensure clinical information in the medical record is complete and accurate reflecting illness severity.
