Job summary

Qualifications

• Must have strong English communication skills (written and verbal).
• Ability to manage multiple concurrent projects and coordinate interdisciplinary rounds.
• Background in clinical care with a focus on patient-centered outcomes.
• Minimum 5 years of clinical experience and 1 year in Case Management (or as UCI employee meeting clinical requirements with a BSN).
• Familiarity with Joint Commission, CMS, CDPH and CCS requirements.

Responsibilities

• Conduct high level assessment and reassessment for patient/family priorities and care planning.
• Coordinate care across interdisciplinary teams and ensure safe, cost-effective transitions of care.
• Document care plans clearly in the medical record and collaborate with physicians to improve clinical documentation.
