Job summary

Qualifications

• Minimum 5 years of clinical experience and 1 year of Case Management experience or a UCI employee who meets clinical requirements with a BSN.
• Excellent written and verbal communication skills in English.
• Current RN License and nursing credentialing requirements.
• Ability to manage multiple concurrent projects and coordinate care transitions.

Responsibilities

• Coordinate high-level assessment and reassessment to plan patient care.
• Collaborate with interdisciplinary team to facilitate safe, cost-effective care.
• Ensure accurate clinical documentation reflecting severity of illness.
