Job summary

Qualifications

• Minimum 5 years of clinical experience and 1 year of Case Management experience
• BSN required for external candidates
• Current RN License and strong communication skills in English
• Familiarity with Joint Commission, CMS, CDPH and CCS requirements

Responsibilities

• Set priorities, plan, organize and implement goal-directed care for patients/families.
• Collaborate with interdisciplinary teams to coordinate care and transitions of care.
• Ensure clinical information in the medical record is clear and complete and aligns with decision support criteria.
• Monitor resource utilization across the health system and evaluate services provided.
