Job summary

UCI Health, part of the University of California, Irvine, seeks an experienced Case Manager RN to lead high-quality, patient-centered care coordination. You will assess and reassess patient needs, plan interventions, and collaborate with an interdisciplinary team to optimize transitions of care across the health system.

The role requires at least 5 years of clinical experience with case management, a BSN, current RN license, and strong communication skills.

Qualifications

• Clinician with knowledge of acuity levels for patient populations.
• Excellent written and verbal communication in English.
• Current RN license and BSN required for external candidates.
• Experience in case management and coordination across teams.
• Familiarity with Joint Commission, CMS, CDPH and CCS requirements.
• Strong computer literacy and documentation skills.
• Ability to manage multiple projects and lead interdisciplinary rounds.

Responsibilities

• Perform high-level assessment and reassessment to set patient care priorities.
• Plan, organize, and implement goal-directed care and transitions of care.
• Coordinate with interdisciplinary team to deliver safe and cost-effective care.
• Advocate for patient self-determination and ensure accurate medical records.
• Monitor resource utilization and ensure compliance with regulatory requirements.
