Job summary

University of California, Irvine Health seeks an experienced RN Case Manager to perform high-level assessment and reassessment to set priorities and plan patient-centered care. You will coordinate with an interdisciplinary team, promote self-determination, and ensure safe transitions of care across the health system.

Candidates should have at least 5 years of clinical experience and 1 year in case management (or be an eligible UCI employee meeting clinical requirements and holding a BSN).

Qualifications

• Must have strong customer service skills and clear English communication.
• Current RN license and a BSN is required for external candidates.
• Ability to manage multiple concurrent projects and lead interdisciplinary rounds.

Responsibilities

• Perform high-level assessment and reassessment to set patient care priorities.
• Coordinate care with an interdisciplinary team to ensure quality, safety, and cost-effectiveness.
• Advocate for patient self-determination and coordinate transitions of care.
