Job summary

Moments Hospice is seeking an experienced RN Case Manager to support patients at home across Madison and surrounding areas. You will assess needs, coordinate care, educate families, and work closely with the care team to ensure comfort in final days and moments.

The role requires an RN license in the state, proficiency with EMR, strong planning and communication skills, and a valid driver’s license with access to a reliable vehicle. A comprehensive benefits package is included.

Qualifications

• Proficient with EMR systems and documentation.
• Licensed RN in the state of practice.
• Strong planning, organizing, and communication skills.
• Ability to solve problems autonomously while maintaining high standards.
• Valid Driver's License with reliable vehicle and current insurance.

Responsibilities

• Conduct initial patient assessments and refine care plans.
• Provide hands-on nursing care and patient education.
• Maintain open communication with patients, families, and the healthcare team.
• Coordinate patient care for caseload and ensure timely interventions.
• Participate in interdisciplinary team meetings for comprehensive care.
