Job summary

Moments Hospice is seeking an RN Case Manager to deliver compassionate, patient-centered care in our Wisconsin service area. You will conduct assessments, coordinate care, and educate patients and families while collaborating with a multidisciplinary team.

This role requires an active RN license, proficiency with EMR systems, and a valid driver's license with insured vehicle. Competitive salary and comprehensive benefits accompany this on-the-road position.

Qualifications

• Licensed as a Registered Nurse in the state you work in.
• Experience with Electronic Medical Records (EMR) software.
• Excellent planning, organizing and communication skills.
• Able to solve problems autonomously while maintaining high standards.
• Valid Driver's License and reliable vehicle with current insurance.

Responsibilities

• Conduct initial client assessments and refine care plans.
• Provide hands-on nursing care and patient education.
• Maintain open lines of communication with patients, families and the healthcare team.
• Manage patient care coordination for your caseload.
• Participate in interdisciplinary team meetings to ensure comprehensive care planning.
