Job summary

Moments Hospice is seeking an RN Case Manager to help make patients' final days with comfort and compassion. You will work closely with the owners in a growth-oriented environment and participate in clinical advancement.

Responsibilities include assessments, care plan refinement, direct nursing care, and coordinating patient care across the healthcare team. A valid RN license is required, along with EMR proficiency and strong communication.

Qualifications

• Licensed registered nurse (RN) in the state you work in.
• Proficiency with Electronic Medical Records (EMR) software.
• Excellent planning, organizing, and communication skills.
• Ability to solve problems autonomously, maintaining high standards.

Responsibilities

• Conduct initial client assessments and continuously refine care plans.
• Provide hands-on nursing care and patient education.
• Maintain open lines of communication with patients, families, and the healthcare team.
• Manage patient care coordination for your caseload.
• Participate in interdisciplinary team meetings to ensure comprehensive care planning.
