Job summary

Moments Hospice is seeking an RN Case Manager to provide compassionate, patient-centered care across our service area. This on-the-road role involves conducting initial assessments, coordinating care plans, and collaborating with the care team to ensure comfort in patients' final days.

Qualifications

• Licensed RN in the state you work in.
• Proficiency with EMR software.
• Excellent planning, organizing, and communication skills.
• Ability to solve problems autonomously while maintaining high standards.
• Valid Driver's License and reliable vehicle with current insurance.

Responsibilities

• Conduct initial client assessments and refine care plans.
• Provide hands-on nursing care and patient education.
• Maintain open lines of communication with patients, families, and the healthcare team.
• Manage patient care coordination for your caseload.
• Participate in interdisciplinary team meetings to ensure comprehensive care planning.
