Job summary

Moments Hospice is seeking an RN Case Manager to provide compassionate, patient-centered care across the care continuum. You will assess needs, develop and refine care plans, educate patients and families, and collaborate with an interdisciplinary team.

This role includes driving to patient locations and coordinating with owners to foster growth and clinical advancement. You will work with a supportive leadership team, benefit from health coverage, paid time off, and a comprehensive compensation

Qualifications

• Registered Nurse licensed in the state you work in.
• Proficiency with Electronic Medical Records (EMR) software.
• Excellent planning, organizing, and communication skills.
• Ability to solve problems autonomously while meeting standards.
• Valid Driver's License and reliable vehicle with current insurance.

Responsibilities

• Conduct initial client assessments and refine care plans.
• Provide hands-on nursing care and patient education.
• Maintain open lines of communication with patients, families, and healthcare team.
• Coordinate patient care for your caseload.
• Participate in interdisciplinary team meetings for care planning.
