Job summary

Moments Hospice in Wisconsin is seeking an experienced RN Case Manager to coordinate patient care across days and nights. You will assess patients, refine care plans, provide hands-on nursing care and education, and collaborate with interdisciplinary teams.

Qualifications

• Active RN license in the state you work in
• Proficiency with EMR software
• Excellent planning, organizing, and communication skills
• Ability to solve problems autonomously, ensuring responsibilities are met while maintaining high standards
• Valid Driver's License and reliable vehicle with current insurance

Responsibilities

• Conduct initial client assessments and continuously refine care plans
• Provide hands-on nursing care and patient education
• Maintain open lines of communication with patients, families, and the healthcare team
• Manage patient care coordination for your caseload
• Participate in interdisciplinary team meetings to ensure comprehensive care planning
