Job summary

Moments Hospice seeks an RN Case Manager to make patients' final days as comfortable as possible. You will work closely with the company owners, supporting entrepreneurial growth and internal advancement.

Responsibilities include initial assessments, care plan refinement, patient education, and coordinating care across the health team. A valid RN license, EMR proficiency, and a driving vehicle with insurance are required.

Qualifications

• RN license in the state you work in.
• Proficiency with EMR software.
• Excellent planning, organizing, and communication skills.
• Valid Driver's License and reliable vehicle with current insurance.

Responsibilities

• Conduct initial client assessments and refine care plans.
• Provide hands-on nursing care and patient education.
• Maintain open lines of communication with patients, families, and the healthcare team.
• Manage patient care coordination for your caseload.
• Participate in interdisciplinary team meetings to ensure comprehensive care planning.
