Job summary

Moments Hospice in Wisconsin is seeking an RN Case Manager to provide high-quality nursing care to patients in their homes and communities. You will conduct initial assessments, refine care plans, educate patients and families, and coordinate with an interdisciplinary team to ensure comprehensive, compassionate care.

This on-the-road role includes a company car with fuel and insurance, competitive salary between $75,000 and $90,000, and full benefits.

Qualifications

• Must be licensed RN in the state of work.
• Proficient with EMR software.
• Excellent planning and communication skills.
• Valid driver’s license and reliable vehicle with current insurance.

Responsibilities

• Conduct initial client assessments and continually refine care plans.
• Provide hands-on nursing care and patient education.
• Maintain open lines of communication with patients, families, and the healthcare team.
• Manage patient care coordination for your caseload.
• Participate in interdisciplinary team meetings to ensure comprehensive care planning.
